Assists the Director of Clinical Services/Administrator with the supervision of home health services to clients in their place of residence in accordance with physician orders and providing the highest quality of care. Supervises skilled nursing and other therapeutic services furnished. The Clinical Manager is available during operating hours (or appoints a similarly qualified alternate) and participates in all activities relevant to professional services furnished, including the development of qualifications and assignment of personnel. This is a safety sensitive position.

  • Understands and adheres to established Agency policies and procedures and uses appropriate channels to facilitate change.
  • Serves as a role model for all colleagues by setting an example of high standards in dress, conduct, cooperation and job performance.
  • Participates in the development of standards, which ensure safe and therapeutically effective service to clients and their families.
  • Makes the initial evaluation visit and regularly reevaluates the client’s nursing needs.
  • Develops, implements, and evaluates individualized client care plans.
  • Provides those services requiring substantial specialized nursing skills.
  • Initiates appropriate preventive and rehabilitative nursing procedures.
  • Prepares clinical and progress notes, coordinates services, informs the physician and other personnel of changes in the client’s condition and needs.
  • Plans for client discharge from services.
  • Completes and submits OASIS assessments, reassessments, and transfers, resumption of care, discharges and significant change in condition in accordance with Agency policies.
  • Ensures POC and tracking system is completed in accordance with Agency policies.
  • Counsels clients and families/significant others in meeting nursing related needs.
  • Refers to Physical Therapist, Speech Language Pathologist, Occupational Therapist and Medical Social Worker those clients requiring their specialized skills.
  • Participates in utilization review of medical records as assigned.
  • Ensures protection of confidential information that is stored, handled or maintained by the agency per HIPAA regulations.
  • Participates in the coordination of Agency’s services with the services of other community agencies.
  • Accepts responsibility for regular attendance and punctuality; fulfills job related requirements without regards to time involved.
  • Develops a cooperative relationship and communicates effectively and professionally with physicians.
  • Investigates and reports any problem relating to client care and/or employee well-being.
  • Immediately reports accidents, incidents, lost articles, or unusual occurrences to the Director of Clinical Services/Administrator.
  • Participates in and presents in-service programs for nursing personnel as needed.
  • Assists Director of Clinical Services in conducting client care conferences, staff meetings, and maintains documentation.
  • Ensures that necessary supplies and equipment are available.
  • Gives total client care as needed.
  • Takes on-call duty nights, weekends and holidays, as assigned.
  • Acts as Director of Clinical Services in the Director’s of Clinical Service’s absence, when assigned.
  • Performs duties following safety standards required for this position including, but not limited to, use of PPE.
  • Assists with supervising, guiding and developing nursing skills and performance.
  • Consults with the Director of Clinical Services/Administrator in determining a staffing pattern which will accomplish stated objectives and promote a maximum level of utilization of health personnel.
  • Assists in recruiting, hiring, evaluating and terminating nursing personnel. Assists in instituting, interpreting and documenting disciplinary action.
  • Responsible for the orientation of new client care personnel as assigned.
  • Assists in the planning and consultative needs of staff.
  • Assists in the preparation and implementation of policies and procedures which meet Medicare, Medicaid, accrediting bodies, state and local laws.
  • Assists in the coordination of interdisciplinary team services, client care and referrals.
  • Assists in the organization of nursing services to delineate and delegate functional responsibility, lines of relationship and communication to provide safe and therapeutically effective service.
  • Participates in the coordination of Agency’s services with the services of other community agencies.
  • Observes confidentiality and safeguards all client related information.
  • Serves as a resource person to employees.
  • Attends pertinent continuing education programs other than routine in services and shares information with staff.
  • Monitors equipment for appropriate use and takes steps to keep misuse to a minimum.
  • Assists with the Agency’s ongoing Quality Assessment and Performance Improvement (QAPI) program.
  • Assures the development, implementation and updates of the individualized client plans of care.
  • Reviews client medical records for compliance with federal, state, accrediting bodies, local and Agency policies and guidelines.
  • Performs nursing functions in times of staff shortage.
  • Ensures that documentation is completed in accordance with Agency policies.
  • Makes staff and client assignments and on-call schedules.
  • Assures that clients’ needs are continually assessed.
  • Attends training sessions required by the company.
  • Advocates for clients’ welfare and serves as a liaison between clients, their families, and healthcare providers (including PCP, supply companies, hospitals, hospices, nursing homes, and health insurance companies).
  • Performs other duties assigned by the Director of Clinical Services/Administrator.

Minimum Qualifications:  

  • High school diploma or the equivalent
  • Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing
  • Two (3) years nursing experience preferred
  • One (1) year experience in home health, preferred
  • Acceptance of philosophy and goals of Agency
  • Ability to exercise initiative and independent judgment
  • Ability to work with individuals to enlist cooperation of many people to perform/achieve a common goal
