Description

Summary: Performs the duties of Licensed Practical Nurse maintaining the standards of care outlined in the Nurse Practice Act by Arizona State Board of Nursing and responsible for performing essential care for residents in our medical Group Homes and Skilled Nursing Facility, and for IHH clients in their homes. This is a safety sensitive position.

Responsible for SNF Patients which includes:

  • Participate in patient care conferences
  • Perform routine nursing duties, including but not limited to:

a) taking and recording temperature, pulse, and blood pressure

b) Comfort, grooming, and hygiene needs

c) Diet management

d) Collecting specimens

e) Patient activity

f) Medication administration

g) Treatments

  • Provide for the safety and emotional and physical needs of the patient
  • Safeguard equipment and supplies, and checks that equipment apparatus or appliances furnished to patient in the course of treatment are in good working order during the patient stay
  • Perform skilled nursing procedures in conjunction with the patient care plan and in accordance with the state Nurse Practice Act
  • Observe, record and report to the Charge Nurse on any changes in the general physical and mental condition of the patient
  • Assist in the health teaching of patients and family members
  • Assist with patient admission and discharge
  • Document required information legibly in medical chart of nursing notes, seizure record, medication record, treatment record, and BM/voiding record, daily assessment, etc.
  • Review information during charting process, making assessments and recommendations for improved care
  • Provide accurate, complete shift reports on written shift report and in verbal reports to oncoming nurse
  • Work with each department to ensure consistency and continuity in the treatment plan by updating care plans as necessary
  • Attend all mandatory nurses’ meetings
  • Must perform duties following safety standards required for this position including but not limited to use of PPE’s.
  • Must attend all training sessions required by the company

Responsible for Group Home Residents which includes:

  • Provide for the safety, emotional and physical needs of all residents
  • Ensure residents are clean, well-groomed and dressed age appropriately and correctly for the season and environmental conditions
  • Position and re-position every 1 hour while awake and every 2 hours while asleep and as needed
  • Help residents cook meals and to eat as needed
  • Ensure daily that residents are being emotionally and physically stimulated to the best of their ability
  • Assist with showering residents to ensure a complete shower is given as needed
  • Responsibility to the D.O.N. and Medical Group Home Manager for reporting all medical aspects of care and changes in conditions regarding the residents in the home.

Responsible for IHH clients in their homes which includes:

  • Perform routine nursing duties, including but not limited to:

a) taking and recording temperature, pulse, and blood pressure

b) Comfort, grooming, and hygiene needs

c) Diet management

d) Collecting specimens

e) Patient activity

f) Medication administration

g) Treatments

  • Provide for the safety and emotional and physical needs of the patient
  • Safeguard equipment and supplies, and checks that equipment apparatus or appliances furnished to client in the course of treatment are in good working order
  • Perform nursing procedures in conjunction with the Plan of Care and in accordance with the state Nurse Practice Act
  • Observe, record and report to the Case Manager or Clinical Director on any changes in the general physical and mental condition of the patient
  • Prepare and submit clinical and progress notes for each patient visit and summaries of care conferences
  • on his/her patients within 48 hours as per Agency policy
  • Assist in the health teaching of patients and family members
  • Document required information legibly in medical chart of nursing notes, seizure record, medication record, treatment record, and BM/voiding record, daily assessment, etc.
  • Review information during charting process, making assessments and recommendations for improved care
  • Work with IHH staff to ensure consistency and continuity in the treatment plan by updating Plans of Care as necessary
  • Attend all mandatory nurses’ meetings
  • Must perform duties following safety standards required for this position including but not limited to use of PPE’s.
  • Must attend all training sessions required by the company

Responsibility to the Company which includes:

  • Being flexible in hours worked and location worked in order to assist with staff coverage if needed
  • Ensuring that you maintain compliance in regards to
  1. Article 9
  2. Prevention and Support
  3. Annual TB skin test
  4. Finger Print Clearance Card
  5. CPR
  6. Annual Flu vaccine (or wearing a mask at all times if on duty if refused flu vaccine)
  7. Other Compliance requirements as directed: e.g.; HIPAA, OSHA, Emergency Plans, etc.
  8. Adhere to and follow all directives and policy and procedures handed down by the company
  9. Ensure that all required documentation is completed before the end of shift.

Qualifications

Minimum Qualifications:

  • Must have and maintain current Arizona State Licensed Practical Nurse license (In good standing)
  • Maintain standards of care as outlined in the Nurse Practice Act by Arizona State Board of Nursing
  • Flexible hours to work times as well as work locations to assist with staff coverage if needed
  • Reliable transportation to work at any location
  • Must have or be capable of passing and maintaining Article 9 and Prevention and Support within 90 days of hire
  • Must have or be capable of passing and maintaining CPR within 30 days of hire
  • Must have or be capable of obtaining and maintaining a Class 1 Finger Print Clearance Card
  • Must be capable of passing a physical & drug screen
