$5,000 Hiring Bonus

Supervises home health clinical services to homebound patients in their place of residence in accordance with physician orders and plans of care and strives to provide the highest quality of care. The Director, Clinical Services supervises all Agency clinical activities relevant to professional services furnished, including the development of qualifications and assignment of clinical personnel. This is a safety sensitive position.

  • Participates in developing standards, which ensure safe and therapeutically effective service to patients and families.
  • Participates in developing clinical objectives for Agency.
  • Has responsibility, under the direction of the Program Administrator, for seeing that clinical objectives are implemented.
  • Assists the Program Administrator in seeing that all clinical professional standards are met.
  • Ensures compliance with all State and Federal rules and regulations and Agency policies in all patient care aspects of the Agency.
  • Assists the Program Administrator in preparing and maintaining current Policies and Procedures which meet State and Federal rules and regulations and implements such; revises concurrently.
  • Displays a willingness to support the Corporate and Agency Policies and Procedures and uses appropriate channels for change of such policies.
  • Implements the Agency’s Policies and Procedures.
  • Participates in studies and research and other administrative functions as assigned.
  • Investigates and reports to the Program Administrator, any problem relating to patient care or conditions which might harm the patient and/or clinical staff well-being.
  • Immediately reports any accident, incident, lost articles, or unusual occurrence to the Program Administrator.
  • Assists with the oversight of the Agency's ongoing performance improvement (PI) program.
  • Assists with overseeing development of indicators with appropriate data collection, aggregation, and analysis, taking action and reporting results according to Agency's PI plan.
  • Assists with the coordination, monitoring and evaluation of contractual services for all services not provided by the home health agency.
  • Audits patient medical records for compliance with State and Federal regulations and Agency policies and guidelines.
  • Assures ongoing assessment of patient/family needs and implementation of interdisciplinary team plan of care.
  • Assures physician approval of plans for continuity of medical care.
  • Ensures that patients’ plans of care are developed, implemented and evaluated.
  • Works with Program Administrator in identifying budgetary requirements and determining appropriate use of allocations.
  • Consults with the Program Administrator to determine a staffing pattern which will accomplish stated objectives and promote maximum level of utilization of health personnel.
  • Is responsible for recruiting, hiring, orienting, evaluating, in-servicing and terminating clinical personnel.
  • Under the direction of the Program Administrator, develops, plans and implements staff training and education and maintains records of such.
  • Participates in establishing the functions and qualifications for each clinical position.
  • Provides individual or group support concerning job related stress or issues.
  • Organizes the Agency’s clinical staff to delineate and delegate authority, functional responsibility, lines of relationship, and communication to provide safe and therapeutically effective service.
  • Oversees staffing and visit assignments.
  • Plans and arranges for consultation needs of staff.
  • Establishes criteria and procedures for selection, promotion, and termination of employment of clinical personnel.
  • Ensures 24 hour/day, seven (7) days/week on-call coverage by scheduling on call RNs for coverage on weekends Holidays, and nights from 5 p.m. to 8 a.m.
  • Under the direction of the Program Administrator, plans and implements marketing strategies for the Agency to hospitals, healthcare plans, physicians, State agencies and the general public.
  • Develops a cooperative relationship and communicates effectively and professionally with the physicians.
  • Conducts patient case conferences, in-services and staff meetings at least monthly and maintains documentation; participates in community education projects.
  • Participates in, and is ultimately responsible for, coordinating Agency's clinical services with services of other community agencies.
  • Observes confidentiality and safeguards all patient related information.
  • Ensures patient appropriate services are provided under the supervision of a registered nurse.
  • Addresses and corrects all patient concerns and immediately reports them to the Program Administrator.
  • Monitors equipment for appropriate use and take steps to keep misuse to a minimum.
  • Ensures that all necessary supplies and equipment are available.
  • Serves as a role model for all colleagues by setting an example of high standards in dress, conduct, cooperation and job performance.
  • Accepts responsibility for regular attendance and punctuality fulfills job requirements without regard to time involved.
  • Serves as a resource person to employees.
  • Attends pertinent continuing education programs other than routine in-services and shares information with clinical staff.
  • Is normally available at all times during and after operating hours; may designate a qualified temporary replacement if he/she will not be available.

Minimum Qualifications:  

  • BSN Required.
  • Must be an RN with a current AZ license in good standing.
  • Minimum of three years nursing experience which includes two years home health experience.
  • Must have prior management experience.
  • Must have an AZ license and must have a current DMV report.
